I grant Camp Bowers (Baby Ballers) and its staff permission to photograph, record, and/or video my child during camp activities for educational, promotional, and marketing purposes. These images and recordings may be used in E3 Camps and/or LBO Sports materials, including but not limited to websites, social media, digital advertising, and printed publications.

Campers must be on the court and ready 15 minutes before start time to allow for check-in and warm-up. Campers must be picked up promptly at the end of each session. Staff are not responsible for supervision outside scheduled camp hours.

In the event of a medical emergency, I understand and authorize my child to be transported to the nearest appropriate medical facility by local emergency personnel if deemed necessary. Any such transport and medical treatment will be billed to my child’s primary medical insurance. I further understand that in certain situations, emergency services may be contacted prior to notifying the parent, guardian, physician, or other authorized contact.

I, the parent or legal guardian of the registered camper, voluntarily grant permission for my child to participate in Camp Bowers and acknowledge that camp activities involve inherent risks, including but not limited to physical injury, illness, accidents, or other unforeseen circumstances. I knowingly and voluntarily assume all such risks on behalf of my child and, to the fullest extent permitted by law, release, waive, and hold harmless Camp Bowers, its owners, directors, officers, employees, volunteers, agents, and affiliates from any and all claims, liabilities, demands, or causes of action arising out of or related to my child’s participation in camp activities, except in cases of gross negligence or willful misconduct. I affirm that my child is physically and emotionally able to participate and that all relevant medical conditions, allergies, medications, or special needs have been disclosed. In the event of illness or injury, I authorize Camp Bowers staff to obtain emergency medical treatment if I cannot be reached and accept full financial responsibility for any associated costs. I agree to indemnify and hold harmless Camp Bowers from any claims or expenses arising from my child’s participation or failure to follow camp rules or staff instructions, and I acknowledge that violation of camp policies may result in dismissal without refund. By submitting this form, I confirm that I have read, understand, and agree to this Liability Waiver and Participation Agreement and that I am legally authorized to consent on behalf of my child for the duration of their participation in Camp Bowers.
